mosaic’s Independent Life Project (ILP) has been a registered Domiciliary Care Agency since November 2004. Services to Young People(SYP) has been delivering services for disabled young people for over 15 years; part of SYP has been about offering a 1:1 service supporting young disabled people to access and use community services.
ColtonCare@mosaic is part of our service development which is looking at working with the Government’s “Personalisation Agenda.”
ColtonCare@mosaic is a development growing out of both ILP and SYP; it will respond to what disabled people want and need out of a personalised, person centred service.
Traditionally, disabled people have had no choice about who provides their care. The Personalisation Agenda is about disabled people having choice and control over the services they use; ColtonCare@mosaic is delivering a flexible service which is tailor-made to suit each individual customer.
ColtonCare@mosaic offers customers the opportunity to negotiate and buy a flexible package of support using Direct Payments, Individual Budgets, Independent Living Fund and Access to Work monies.
ColtonCare@mosaic offers personal and domestic care in the home, support to use community and leisure, educational and social facilities (combined with personal care is necessary). We work with people with physical and sensory impairments, learning disabilities, people who experience mental ill health, older people and people with long term health conditions.
